Basic Fertility Workup
|
After first consultation with cou0ples we assess potential reason for failure to conceive. We investigate both husband and wife to find the cause of infertility. Investigations include – blood tests, husband’s semen analysis, ultra sound and other imagings, if needed.
We meet the couple again with all results to make the best suitable treatment
|
Ovulation Induction and Follicular Monitoring
|
Once we have agreed treatment plan we need to help ovaries of female partner to produce eggs. Normally one egg produced each month. During IVF Treatment we make ovaries super ovulate i.e. more eggs are produced to enhance chances of conception. For this purpose various combinations of medicines which are injections are used.
Once ovaries are stimulated we need to watch them carefully for size of eggs. This is done by serial ultra sound scans. The eggs need to be taken out on very precise moment and that is the time when we get husband’s semen sample and actual process of fertilisation begins.

Sperm Processing for Intrauterine Insemination (I. U. I.)
|
Semen samples are processed by washing, centrifugation and migration with suitable culture medium requiring CO2 incubation. The spermatozoa thus obtained are free of debris and bacteria, seminal
fluid, are energetic and improve the success rate of intrauterine insemination.
We have special male room for semen collection. We also prepare enriched samples
for other clinics so that they can perform intrauterine insemination.
|
In Vitro Fertilisation (IVF)
|
In I.V.F., pregnancy is achieved by fertilizing the woman’s eggs (oocyte) outside her body. The oocytes are obtained under mild anaesthesia through vagina using transvaginal U/S guided sonographic procedures. Semen is processed in order to harvest the most active sperms. The eggs and sperm are incubated together. They are checked for fertilization. Embryos are ready for transfer the next day. IVF is ideal for couples with tubal blockage or pelvic adhesions, previous tubal sterilization not amenable to reversal, ovulatory disorders, unexplained infertility, and some cases of endometriosis.
|
Micromanipulation / Intra Cytoplasmic Sperm Injection (I.C.S.I)
|
In this procedure, oocyte is prepared by removing its
outer coat of cells called the cumulus complex. Once laid bare, the oocyte is held
gently by a holding pipette under an inverted microscope. The sperm that has been
prepared for microinjection is injected through a very fine micropipette into the
cytoplasm (body) of the oocyte. The holding pipette is 20 times thinner than human
hair and the injecting pipette is 60 times thinner than human hair. The egg itself
is no bigger than the tip of a pin. The microinjected egg is returned to the incubator
and checked the following day for fertilization. Micromanipulation is required for
couples with very low sperm counts or zero count (azoospermia), very little or complete
lack of motility and abnormal shape of the sperm. Its use is extended to women with
advancing age, women suffering from endometriosis and those with immunological and
unexplained infertility.
|
Testicular & Epididymal Sperm Aspiration (TESA & PESA)
|
This technique is used in cases where the husband is having difficulty in producing sperms or no sperms are found. In this technique sperms are retrieved surgically under pain free procedure. After getting the sample sperms are assessed and then frozen for further use in fertilisation.

Donor Insemination Programme, Semen Cryopreservation and Banking
|
Our sperm bank uses donor semen, which has undergone stringent evaluation. The donors are tested for HIv, Hepatitis, VDRL, and Gonorrhea. We follow the W.H.O. criteria to ensure semen quality. The Cryopreservation Donor Semen Prgramme is in accordance with the guidelines established the British Fertility Society. We routinely have a backup semen sample frozen for couples who are undergoing treatment in case
of inability to give a sample on the day of the IUI, IVF or ICSI. The men who are
undergoing orchidectomy and cancer treatment before they have radiation and chemotherapy
can also use our cryo preservation facilities. We routinely cryopreserve testicular
and epididymal sperm, so that repeated attempts at obtaining fresh epididymal and
testicular sperm are avoided.
|
Embryo Cryopreservation
|
We offer facilities to cryopreserve surplus embryos for future use. Our freezing
protocol is completely standardized. All embryos are coded, labelled and stored
in special containers for easy and fool-proof identification.
|
Donor Oocyte Programme
|
We have programme for Oocyte donation for women with premature ovarian failure,
surgical or induced menopause and those in the perimenopausal group. Some women
with repeated poor recruitment of follicles at the time of I.V.F. , women
with extensive endometiosis, severe pelvic adhesions and inaccessible ovaries might
also qualify.
|
Assisted Hatching
|
| Assisted hatching is a procedure in which the zona (shell) of the embryo is opened using techniques of micromanipulation. This facilitates its hatching in the uterus and improves implantation and pregnancy rates. Assisted hatching is useful for older women where the zona tends to thicken and in women who have tried IVF unsuccessfully.

Counselling
|
Infertility and its treatment caused significant stress to couples, which could
be detrimental to the reproductive cycle. Before definitive treatment begins we
encourage couples to talk about their stress factors and initiate counselling
is manadatory. In addition, we have a Patient Support Group, which offers regular
information on all aspects of treatment. This is an opportunity to meet other couples
who are going through treatment. Many couples find this of enormous benefit.

'Open House'- Infertility Awareness Programme
|
We are running 'An infertility awreness programme' for infertile couples in Hindi
and English. This programme is an "open house" for infertile couples and others
interested in infertility. Infertility could be devastating experience. In this
programme the couples can take part in discussion with infertility specialists,
Dr. Anita Gour, Dr. Ramdoss Srinivasan and their team. This programme
aims to clear the misconcepts and myths about infertility and give recent scientific
knowledge available in the field of infertility. For further information about the
services or any of the specific procedures offered by Genesis Fertility Centre or
to reserve space for the Open Evenings or Patient Support Group please do not hesitate
to contact us by phone or e-mail.
